I had something similar and had to take to the shop, the used some tools to measure and had to straighten things some... only happened when climbing hills, ect...

Yeah, probably a tweaked hanger.

How long have you been riding this chain + cassette? It could be worn out and struggling. If it is just the derailleur shifting issue then the cable tension may not be right. Can you describe the issue more. is the problem only shifting to your lower/bigger gears or is the issue only when you put pressure on the chain with your body weight?

Is it changing gears, or just jumping off/on the same gear? If it's the same gear, then it's most likely a worn cassette or chain. That seems to happen to me at least once a year...

In addition to verifing the hanger and life left in it I would also take a look at cable stretch. The park instructions are pretty good for getting it right the first time. If you test it while riding it and applying a bit of brake you can typically emulate a hill...

One of the best few dollar investments you can make is for a chain wear checking tool. Tell ya right away if a chain's still got life in it or if it's stretched and wearing on your cog/chainring in a bad way. If you change your chains in a timely manner when they start to stretch, the rest of your drivetrain will last much longer (and save you a buncha $$).
